Nominees must be currently employed by an educational institution in Ohio (K-12 or ESC).

How to submit an application:

All the required documents must be submitted as a single .pdf file, emailed as an attachment to awards@ITIPOhio.org with “Nomination for 2015 ITIP Ohio ISTE Award” in the subject area of the email.

The following items must be submitted in one PDF document:

1. Complete nomination form (one person per nomination)
2. An application narrative, developed by responding to the five (5) short answer questions, listed in the rubric (below), describing the work of the nominee as it applies to the appropriate award category and rubric.  Narrative must be limited to 2 single-spaced pages in 12 point font.
3. Two letters of recommendation – one from an administrator and one from a peer.

In submitting an application for the Outstanding Technology-Using Teacher or Administrator of the Year, a scoring rubric outlines evaluation criteria which should be specifically addressed within each application.  A committee will review the applications based on these criteria.

While nominations can touch on accomplishments that occur over the course of a nominee’s career, the primary focus of the nomination should be on the work and accomplishments that occurred during the current (2014-2015) and prior (2013-2014) years.

Winners receive great prizes including free registration to the Ohio Educational Technology Conference (OETC), ISTE Membership, an ISTE “Making IT Happen” jacket and free registration for the 2015 ISTE National Conference in Philadelphia.  The winner of ITIP Ohio’s award is entered into ISTE’s international award program (www.iste.org).
